リンガル治療では装置が見えないということに加えて、フォースベクトルから有利な点も多い。しかし、その装置の構造から生じる問題点もあるのが事実である。そこで舌側矯正に携わる歯科医なら誰もが活用できるようなアライナーやラビアル治療と融合させつつ、治療の効率化および効果的な手法ならびにオーソアンカーとの併用により、リンガルブラケット矯正法の臨床的効果を引き出せる方法について多くの臨床例をもとに解説する。

Since the introduction of the modern orthodontic bracket by Dr. Angle, there has been great progress in orthodontic treatment using fixed appliance. Unfortunately, the major components of fixed appliances including brackets, bands, and archwires are attached to the labial and buccal surface of the teeth, which makes conventional orthodontic appliances very unaesthetic, but esthetics improved in the 1970’s with the introduction of appliances that used a direct bonding system. Orthodontic bands for brackets were discarded, especially in anterior and premolar segments, however, labial appliances still had esthetic limitations. So, Dr. Fujita and Dr. Kurz have developed the lingual bracket appliances and techniques in the middle of 1970s.
Dr. Angle first introduced important 2 kinds of orthodontic brackets, vertical slot (ribbonwise) & horizontal slot (edgewise) ones. Nowadays, most of orthodontists tend to use edgewise brackets instead of ribbonwise ones. However, there are so many different kinds of lingual brackets with vertical slot and/or horizontal slot are using in daily practice.
I tried almost all kinds of lingual bracket since 1986. So, in this presentation, I am going to share my experiences and knowledge in dealing with different kinds of lingual brackets. Also, I am going to introduce newly developed lingual brackets which was designed by myself as well.

The reasons for a gummy smile include excessive maxillary vertical growth, over-eruption of the maxillary incisors, incomplete anatomic crown exposure, hyperactivity of the elevator muscles of the upper lip, or a combination of these factors. A gummy smile of skeletal origin requires orthognathic surgery for correction. Dentoalveolar gummy smiles caused by overeruption of the maxillary incisors can be corrected by intruding the extruded maxillary incisors. The dentogingival type of gummy smile is related to abnormal dental eruption or lack of gingival recession and requires lengthening of the anatomic crown. The neuromuscular type of gummy smile is caused by hypercontractibility or excessive muscle contraction and can be improved temporarily by injecting botulinum toxin type A.
With the advent of temporary skeletal anchorage devices (TSADs), the conventional concepts of anchorage control and biomechanics in orthodontics have changed, and many case reports have demonstrated orthodontic correction of gummy smile. Most studies have demonstrated management of gummy smile in the anterior area and facial profile in extraction cases by performing intrusive retraction of the maxillary anterior teeth with labial appliances and TSADs.
Considering the center of resistance and the forces applied, this presentation describes the orthodontic impaction method of the maxillary whole dentition to improve gummy smile in both the anterior and posterior areas and facial profile in a nonextraction case with the use of lingual appliances and one midpalatal TSAD.
